FRAIL & ELDERLY SERVICES COMPONENT:

Our frail elderly component offers case management, outreach, linkage to needed services and
van services to the frail, home-bound seniors of this area.
For the frail home-bound elderly we provide assistance in obtaining services such as home-care,
meals on wheels, adult day care and weatherization. We service zip codes 14207 - Black
Rock/Riverside, 14223 - Town of Tonawanda, and 14072 - Grand Island. Our case work
service is no cost; however, many other services such as In-Home Health Care, Personal
Emergency Assistance Systems, Adult Day Care and Meals On Wheels have cost share fees
based upon income.

The following services are offered through a referral to the Erie County Department of Senior
Services. We provide help with completing forms, understanding correspondence and
determining eligibility from various entitlement programs:
Government Programs: Medicare, Medicaid, Food Stamps, Social Security, Subsidized
Housing, H.E.A.P. (Home Energy
Assistance Program), E.P.I.C. (Elderly Pharmaceutical Insurance Coverage - Prescription
Drug Plan.)
Community Based Services: Meals On Wheels, Home Health Care, telephone assurance,
senior companions and grocery delivery.
Referrals are accepted from any source 8:30 AM - 4:30 PM, Monday - Friday. Contact
