Medical Services
This is a group practice. The doctor, physician assistant or nurse practitioner that you
choose will coordinate your care. We have the following special services:

Financial Information
Insurances
We accept most standard medical and dental insurances, as well as Medicaid and
Medicare. At the time of scheduling your appointment, please tell us what type of
insurance you have. It is important that you bring all necessary forms and insurance
cards with you to the Health Center.
Fees
Our fees are reasonable. If you have no medical or dental insurance, please call
prior to your appointment to talk about our sliding fee scale. You may be eligible for
reduced rates based on your family's size and income. All payments and
co-payments are expected at the time of service.
Prescription Refills
If you are on any medication, please plan ahead to be sure you have enough
medicine to last until your next visit. Make sure you know all your medications or
bring all the bottles to your appointment. If you run out, please call us at 875-2904
X273 for our prescription refill hotline.
Emergencies
The Health Center has 24-hour medical emergency coverage. If you
become ill after hours, call the Health Center at 875-2904 and tell the
answering service that you need to speak to your physician.
Hospitalizations
Prior authorization by your physician is needed before hospitalization or emergency
room visits. If you have a non-life threatening problem and must go to the emergency
room, call the Health Center and your insurance as soon as possible.
If you or your family member needs to be hospitalized, your physician will continue
to care for you.
At The Time Of Your Visit...

  • Plan to arrive a few minutes early
  • Bring in all current medications
  • Bring all insurance information and card
  • Bring in any medical records you have, such as immunizations or labs.
  • Tell us about an other doctors you go to
  • Tell us if your visit is a job-related injury, an automobile accident, or a disability Your
    insurance company may require you to list your primary medical doctor with them up to
    a full month BEFORE the first visit. Please call your insurance company to register the
    doctor you have chosen here at the Health Center.
Information about Northwest Buffalo Community Health Care Center
Since 1987, we have been providing services to the Buffalo community. We are
physicians, physicians assistants, nurse practitioners, nurses, medical assistants, a
social worker, a registered dietitian and dentists, a dental hygienist, dental assistants
and many other good people. Together with you and your family, we are a team
offering the best quality health care available.

Patients are seen by appointment. We are not a walk-in clinic. When you call for your
appointment, you will be asked several questions to help determine how much time
you need and whom you should see. If you are unable to keep an appointment, please
call us at least 24 hours in advance.
About Phone Calls...
If you have an urgent problem, please call us as early as possible after 8:30 a.m. The
medical staff will return a call received before 4:00 p.m. on the same day.
Northwest Buffalo Community Health Care Center is located at 155 Lawn Avenue in
the former School #79 building, off of Military Road, one block of north Hertel
Avenue.
